Documentation of practicum required by the MA ESE to be retained in the student's permanent SEAS file includes the following:

All the above forms are available on the SEAS website at http://www.bridgew.edu/LicensureFieldPlacement .  Completed practicum forms and the candidates' request for BSC recommendation for educator licensure should be turned into SEAS at program completion.  Student files and documents are subject to audit by the MA ESE during program review visits.

Application for educator licensure is submitted via the MA ESE Educator Licensure and Recruitment portal www.ese.mass.edu/elar.  If you have not done so already, candidates set up an on-line educator profile with the MA ESE.  The site will "walk" you though the process.  Once the profile has been established, educators may apply for new licenses, renew existing professional licenses, etc. using this site.   Candidates may apply for their license at any time during their program.  At the candidate's request (see form) BSC will electronically (ELAR) recommend them for the initial Instructional Technology license.  To complete your application at the MA ESE, the candidate will have to submit a final official transcripts from their undergraduate institution verifying baccalaureate degree conferral and from BSC verifying licensure preparation. 

For School of Graduate Studies forms, students need to pay attention to following forms for graduation:

  • Comprehensive Examination Request - It is the graduate student's responsibility to see that this form is completed and filed with the Graduate Studies School no later than October 1 for Fall exams and February 1 for Spring exams.

  • Application to Graduate - for Graduate students who are nearing the completion of their program requirements and plan to receive a master's degree.
