To request an official transcript of your entire academic record from Bridgewater State University, a written request is required. You may use the link for the Transcript Request Form, or write a letter that contains the following information:
- All previous names under which you may have attended
- Banner ID or last four digits of your Social Security number
- Year of graduation, or year(s) of attendance
- Daytime telephone number
- An email address so that we can contact you with any questions
- Address to which you would like your transcript mailed
- Your signature (by law, transcript requests cannot be processed without the signature of the student)
Pre-pay transcript fee:
All mailed or faxed requests must include payment for the fee of $5 per transcript or you may go to Prepay Transcript Fee to pay by credit card and include the receipt with your Transcript Request Form. Checks should be made payable to Bridgewater State University. Payment by phone is available during business hours (9:00am - 4:30pm) at 508.531.1225. Please do not send cash.

On-the-Spot transcripts may be requested in person during regular business hours. The fee for on-the-spot transcripts is $10 per copy. (Note: All transcripts of graduates prior to 1990 may not be available for “on-the-spot” processing.)
Between 9am - 4:30pm, prepay first in the Office of Student Accounts, Boyden Hall, Room 107. Bring receipt to the Registrar's Office, Boyden Hall, Room 003, with completed Transcript Request form.
Between 8 - 9am and 4:30 - 5pm, please bring a completed Transcript Request Form to the Registrar's Office, Boyden Hall, Room 003 with a prepaid receipt or a check made payable to Bridgewater State University. The Registrar’s Office cannot accept cash or process a credit card. The Registrar’s Office is open for extended hours, until 6pm, on Tuesdays and Wednesdays when classes are in session.
