Acceptance Forms

Congratulations on your acceptance to The College of Graduate Studies at Bridgewater State University.  Along with your acceptance letter you should have received an "Acceptance of Admission" form.  This original form, with ink signature, needs to be returned to The College of Graduate Studies within ten business days. Below you will find a list of additional forms and documents with instructions on how to proceed as a newly admitted graduate student. To view certain files, you will need Adobe Reader. If you do not have Adobe Reader installed on your computer, you can download it for free.

• External Transfer Credit Form (PDF) (Only required for students who have taken courses at institutions other than Bridgewater State University)

• Health Record Form  (Must be returned to the Health Services Department)
• Graduate Student Handbook (It is the responsibility of the graduate student to read and understand all material in the Graduate Student Handbook)
