- Please direct all washer/dryer concerns to Mac-Gray at 1-800-MAC-GRAY (622-4729).

- Please direct all vending/soda machine concerns to Administrative Support Services at ext 2497.

Description: The description should include: item in the room/hall that is having a problem and
a brief but detailed description of the problem. (200 Characters Maximum)

* Important Note: The problem type you selected above in the Problem Type box
does not appear when our office receives this work order. This means, if you just write
"broken" in the box below, we will not be able to determine what is broken. Always
indicate the item that has the problem below.
