Step 5 is the final stage of the CAP Cycle and includes Unannounced Observation #2 and the Summative Assessment, the final task of the CAP Cycle. The PS and SP take a deep dive into all evidence that has been collected in the semester. They jointly determine the rating for each of the Seven Essential Elements based on their collective professional judgment and a thorough examination of evidence that demonstrates the candidate has met all readiness thresholds and is prepared to have positive impact on students. The final three-way meeting is conducted by the PS and SP to share the summative assessment findings with the TC. 

In CAP Step 5, there are three categories of resources from DESE

1. Required Forms

  • CAP Form
    This is the final time you will fill out parts of the CAP Form. In Step 5, the PS, SP and TC all sign and date the paper CAP form at the close of the Three-Way Meeting #3. The PS will also note the date of Three-Way Meeting #3 in the CAP Online Portal. The PS and SP must also determine if the TC is “Ready to Teach”. These tasks signify the end of Step 5. 
     
  • CAP Observation Form
    The PS and SP can use this paper form to capture notes from the Observations conducted to date and prepare feedback for the TC. Unannounced Observation #2 is conducted as part of Step 5. This observation evidence must also be entered into the online CAP Portal*. 
     
  • Summative Assessment Form
    This form is used by the PS and SP to organize and calibrate on findings from the all evidence collected throughout the semester including observation evidence, work toward the professional practice goal, student feedback, impact on student learning, conversations with the TC, written products, and overall classroom performance.  Note that this form looks like the baseline and formative assessment forms. The PS and SP will also need to enter this information into digital form in the CAP Portal*. 

    *We encourage everyone to keep a separate copy (digital or paper) of observation notes and feedback to the TC on the off chance that the data is not saved in the CAP Online Portal for reasons such as loss of Internet connection while completing the online form.

2. Recommended Forms

Step 5 has one Recommended Form:

Post-Conference Planning Form

This form will help the PS and SP organize feedback to the TC after Unannounced Observation #2. This form helps the PS/SP to be sure their conclusions and suggestions are rooted in evidence they collected from the observation. Note that there are not pre-conferences for Unannounced Observations.

3. Suggested Resource
