Student Behavioral Concerns
Community Standards Referral Form
This form is used to report suspected violations of the Student Code of Conduct and other behavioral concerns.
Community Standards Appeal Request
This form is used to appeal decision outcomes following an Investigation by the Office of Community Standards. Information about the Student Code of Conduct and Community Standards procedures may be found in the Student Handbook.
Equal Opportunity/Title IX Complaint Form
This form is used to report information necessary to initiate an investigation of alleged discrimination, harassment, sexual or gender-harassment, sexual assault or other sexual violence, stalking or retaliation pursuant to the Equal Opportunity, Diversity, and Affirmative Action Plan ("EO Plan") and/or the Student Code of Conduct.
CARE Team Referral Form
This form is for faculty, staff and students to voluntary report students who are experiencing a crisis, displaying odd or unusual behaviors or engaging in other behaviors that may be perceived as being harmful to themselves and/or others.
Academic Integrity Policy Violation Report Form
This form is used to report suspected violations of the BSU Academic Integrity Policy.
Classroom Conduct Report Form
This form is used by faculty to report inappropriate classroom behaviors that may result in the exclusion of a student from the classroom by their instructors. Read the Classroom Conduct Policy
